If your young child has a cavity and you are hoping for an option that does not involve drilling or numbing shots, silver diamine fluoride may be part of the answer. Often called SDF, it is a liquid brushed onto a decayed area of a tooth to stop the cavity from getting worse. It has become a widely used tool in pediatric dentistry, especially for toddlers and children who are not ready for a traditional filling.
At Kidz Modern Dentistry in Laguna Niguel, Dr. Lida Pishbin and Dr. Solmaz Shir use SDF when it fits the child and the family’s goals. Every plan starts with a careful exam and an honest conversation about what SDF can and cannot do, including the staining it leaves behind.

SDF is a clear liquid that combines silver and fluoride. The silver kills the bacteria that cause decay, and the fluoride helps harden and strengthen the weakened tooth structure. Together, they can stop, or arrest, an active cavity so it does not keep growing.
According to the American Dental Association, SDF is cleared by the FDA for treating tooth sensitivity, and its use to stop cavities is an off-label use supported by clinical guidelines. It is a painless, non-invasive way to manage decay while preserving natural tooth structure.
For families coming from communities like Aliso Viejo, Mission Viejo, or Laguna Hills, all appointments take place at the Laguna Niguel office on La Paz Road.
The appointment is short and simple, which is a big reason it works so well for young children. There is no drilling, no vibration, and in most cases no numbing shot.
A typical visit goes like this:
The amount of fluoride used to treat a tooth with SDF is less than what is in a standard fluoride varnish application.
The application itself takes only a few minutes per tooth, so it is manageable even for a toddler who cannot sit still for long.
SDF is not the right choice for every cavity, and the decision is made by the dentist after an exam. It works best on cavities that are active and easy to reach, most often in baby teeth.
The American Academy of Pediatric Dentistry supports SDF as part of an ongoing plan to manage cavities in children, alongside regular monitoring.
Children who often benefit include:
Toddlers and preschoolers who are not ready for a traditional filling
Children with dental anxiety or a past difficult dental experience
Children with special health care needs for whom fillings are hard to tolerate
Children with several cavities that cannot all be treated at once
Children for whom sedation is not a good option right now
When a child needs extensive treatment that SDF cannot handle, pediatric sedation may be discussed so the work can be done comfortably in fewer visits.

The biggest trade-off is appearance. SDF permanently stains the treated decay a dark brown or black color. Healthy enamel around it is not stained, but the arrested cavity will stay visibly dark, which matters most on front teeth.
If SDF touches the gums, it can cause mild, temporary irritation, which careful application helps prevent. Some children notice a brief metallic taste. SDF also does not rebuild lost tooth structure, so a tooth with a large cavity may still need a restoration later to restore its shape and function.
If the staining is a concern, especially on a front tooth, a tooth-colored filling can often be placed over the arrested area later on.
A traditional filling removes the decay with a drill and replaces it with filling material. It restores the tooth’s shape and appearance, but it requires the child to sit still, usually involves numbing, and may need sedation for very young or anxious children.
SDF does not remove any tooth structure. It stops the decay where it is, which makes the visit faster and much less stressful. The trade-offs are the dark stain and the fact that the tooth is not rebuilt. The two are not competing options, and many children benefit from both.
A common approach is to use SDF to stop decay now, then place a filling or, for a badly damaged baby tooth, a pediatric crown once the child is older and more ready for treatment.
Baby teeth hold space for the permanent teeth, help children chew comfortably, and support clear speech. An untreated cavity can cause pain, trouble eating and sleeping, and infection, and losing a baby tooth too early can affect how the permanent teeth come in.
Once a cavity reaches the softer layer under the enamel, it tends to grow faster. Stopping it early with SDF can keep a small problem from turning into a painful one that needs more involved treatment. Regular checkups let the dentist confirm the treated areas stay hard and reapply SDF when needed.
Fluoride, sealants, and good brushing habits at home are the best way to prevent new cavities, and the practice’s preventive dentistry services help keep your child’s teeth healthy between visits.

Common questions parents ask, answered by our pediatric dental experts.
The AAPD has reported no known systemic or serious adverse effects from SDF when applied according to manufacturer directions. The amount of fluoride delivered per application is actually less than that contained in a standard fluoride varnish treatment. [2] Clinical trials involving preschool-age children have demonstrated SDF's safety profile when used appropriately. As with any dental treatment, the decision to use SDF for a very young child should be made by a licensed dentist who has evaluated the individual patient.
No. The dark discoloration that occurs on arrested caries lesions after SDF treatment is permanent. Healthy tooth enamel is not stained, but the treated areas of decay will remain visibly darker. This is an important consideration for front teeth. In many cases, a tooth-colored restoration can be placed over the arrested area at a later appointment to improve aesthetics. [1]
Current clinical guidelines from the ADA recommend biannual (every six months) application of 38% SDF for sustained caries arrest on primary teeth. A single application alone has been found insufficient for ongoing benefit, and the treated teeth need regular monitoring by a dentist. [4]
SDF has demonstrated effectiveness primarily on cavitated carious lesions in primary (baby) teeth, particularly on coronal surfaces. Its use has also been studied for root surface caries. It is most often recommended for lesions that are active and accessible for direct application. Your child's dentist will evaluate which specific lesions are appropriate candidates after a clinical examination.
Most children do not experience pain during SDF application. There are no needles involved, no drilling, and no tissue removal. Some children may notice a brief metallic taste. The procedure is generally well-tolerated even by children with significant dental anxiety, which is one of the key reasons it has gained acceptance in pediatric dentistry. If your child has sensitive teeth or exposed dentin, the dentist will take precautions to minimize any discomfort.
SDF can arrest active decay and halt its progression, but it does not rebuild lost tooth structure. If a cavity has caused significant damage to a tooth's form or function, a restoration such as a filling or crown may still be needed at some point. SDF is best understood as a caries arrest tool that can buy time, minimize treatment burden, and often be used alongside restorative care as part of a comprehensive plan developed by your child's dentist. [1]
Coverage for SDF varies by insurance plan. The ADA has formally resolved that it supports SDF as a covered benefit by third-party payers, and also supports coverage for restorative treatment or extraction of a tooth that has previously been treated with SDF. [1] Parents should contact their specific insurance provider to confirm coverage details before scheduling treatment.
